Who Is Eligible To Receive Services?
Douglas Gardens Community Mental Health Center provides mental health services to individuals age 18 and older who live on Miami Beach and in the greater North Miami area (Service zip codes: 33138 (north of 79th street) 33139,33140,33141,33147 (north of 79th street), 33150, 33154, 33161, 33167, 33168, 33169, 33181). A staff member is available to help you to determine if our services best meet your needs.
What Is The Cost Of Services?
At Douglas Gardens, the cost of services will be based on your ability to pay. Insurance such a Medicare, Medicaid and some private insurances are accepted and do cover for services. Individuals who are seeking to qualify for discounted fees under the sliding fee scale must bring proof of income or proof of public assistance. A request for a fee waiver/reduction can be made available if special life circumstances make it difficult for you to afford the reduced fee cost. You will be informed about the cost of treatment prior to the initial assessment. In general, payment for services is expected at the time of your appointment.

What To Bring To Your Appointment
- Identification- Florida State ID, Florida Driver’s license, Permanent Resident Card or passport.
- Social Security card
- Proof of address- A copy of your lease, a piece of mail with name and address listed, an FPL bill/phone bill or a letter from someone residing at the address who attests to your residency could all be forms of proof.
- Proof of Income- The last 4 consecutive check stubs or other proof of income for wage earners in the family unit is requested. Additional proof could include: Unemployment compensation, Worker’s compensation, Social Security award letter, recent income tax return, bank statements for the last two months or proof of income for someone who may be providing support.
